I got click-baited to a survey creator website. I thought I could take a survey to measure my concern, the promotion images were showing survey questions and answers suggesting that I could take a survey, but it's no survey... I still didn't find any viable survey for self to take to see whether I should be concerned. Other use case, if I was feeling sick, I would like to be able to find a survey about my symptoms to help me with a decision to go to the doctor, stay home or check out recommended tailored resources. News is full of panic and warning but no resources like a simple Q&A and action recommendations.

@dmxt https://carbonhealth.com/ has exactly what you're describing. It's of course linked to their services, but you can also just take the survey without any other obligations. Their 'virtual care' works well too if you want to talk to an actual doctor afterwards.
