It is usually in excel or notion. It actually can be tedious sometimes to maintain in excel.

I use Notion for my work journal and it's been great! Super flexible and you can customize it to your needs. I have sections for daily notes, meeting notes, project tracking, and more. The database features are especially handy for relating different types of info together. Worth checking out if you want something more powerful than a plain Excel sheet.
