Great app and easy-to-use UI. Bugs detected: 1. When the modal is closed and reopened, the content remains but the title no longer. 2. And when I tried to change the type of feedback the modal closed. It's probably a click propagation problem. Something like click.self somewhere in the selector might fix it.
