@welly_mulia Let me try again. It took me about a day all together split up over a couple days (including writing and adding in the content and getting feedback). I used https://tally.so which is a form builder that is "notion-inspired". Super cool tool, built by @marie_martens, highly recommend trying it out! They have a great free plan too.
